I started the process of filling out the forms to send to the KCC. Although I find them to be rather straight forward-ish, there are the odd questions that I am unsure of. With this process being as important as it is, I do not want to risk any misunderstandings on my part.
Any help / clarity on some of these questions would be greatly appreciated:
DSP-122 (http://www.state.gov/documents/organization/80148.pdf):
Question 1: Name (Last, First, MI)
Do I have to complete my wife's maiden name here? Or only her current married last name? On the other form (DS-230) they ask for a spouse maiden name - I am not sure how the names should match up.
I assume I have to put her current married last name, her first name and all middle initials - is this accurate.
Question 6a: Education - Check the highest level of education completed:
a) High school, no degree
c) Vocational school
d) College, No degree
e) University degree
f) Advanced degree
If the applicant has only a high school matric certificate, which one is it, a) or
My assumption here is to answer
Question 6b: Names and addresses of all schools, colledges, universities attended:
Should the applicant include all schools attended, incl. primary and high schools? - Or only the high school in which she matriculated?
Should the applicant include the universities which she attended, but did not complete> In other words she studies at University of Pretoria, but she did not complete her degree. Should she include this?

Question 10: Permanent address in the US.
I have no idea! I know we will stay somewhere in Orange County - but that is it. What do I put here or do I leave it blank or do I put N/A?
Question 11: Address in the US where our GC's will be mailed
Can this address be the company's address which I will work for? Nobody lives there, so what would I put down for the name of the person who currently lives there?
Question 14: Spouse's maiden or family name.
My wife has has had my last name for 5 years. Should I put her maiden last name or her current last name?
Question 31: Persons in 14 and 29 accompanying you to the US:
This relates to the previous issue (question 14). If I put my wife's maiden name down for question 14, then I have to list her with her maiden name here. This does not make sense to me since she has my last name for the past 5 years.
Question 33: List all educational institutions attended.
Do I have to list all the schools I attended - primary and high schools? Or do I list all educational institutions attended after school? Do I list an institution even if I did not finish my degree?
Question 39: Name and address of petitioner.
Is the petitioner the DVSelectee? If so then I have to put my wife's details down as she is the selectee.
As you can see, I seem to be struggling with pretty straight forward questions. I just want to be absolutely sure that I am spot on with my answers. How strict are they with these forms? Anyone with any mentionable experience around the form content and submission out there?
Any tips and advise would be greatly appreciated!
